Requirements for Initial Licensure in Northwest Territories, Jan. 1, 2004

Candidates for licensure in the Northwest Territories must present:
- completed application
- completed application form supplied by the Department of Health and Social Services
- a certificate of good standing from a Canadian Provincial Regulatory Authority
- certified true copy of pharmacy degree
- certificate of qualification from the Pharmacy Examining Board of Canada
- current detailed résumé (including particulars as to location and dates of practise history)
- 3 letters of reference on "Reference" form supplied by the Department of Health and Social Services
- copy of birth certificate or immigration documentation

Licensing Fees
- Prior to licensure - n/a
- For initial licensure - $250.00 (yearly from April 1 to March 31)
- Temporary permit (6 months) - $150.00 (available one time only)
